Salary Range

$2,099.00 - $3,041.00 (Salary Range 1-15/Edition No. 11) with BENEFITS

Job Summary

Under the direct supervision of a Family Community Partnership Specialist, the Family Service Associate is responsible for linking families with services that are needed, supporting family independence and competence toward self-sufficiency, with the overall goal of improving their quality of life.

  • Recruits and enrolls children into the program, ensuring full enrollment at assigned sites;
  • Completes enrollment folders and assists in obtaining necessary records such as physical and dental records of children;
  • Assists in the review and analysis of health screening results and processes referrals for further diagnostic testing;
  • Makes home visits;
  • Implements a partnership between family and the Agency;
  • Provides families with emergency assistance and crisis intervention;
  • Conducts family assessments to provide necessary services and follow-ups;
  • Participates and engages in parent conferences;
  • Prepares and submits periodic reports;
  • Serves as liaison between the Agency and community-based organizations and resource agencies;
  • Attends and participates in meetings and in-service training; and
  • Performs other job-related duties as assigned.

Requirements

Must be 18 years of age or older; must be a high school graduate or have completed GED; completion of some coursework in child development or social work preferable; must be able to read and write English; two years of documented paid or volunteer experience working with low-income groups; experience in community outreach, community health and family-focused intervention; ability to work with an ethnically and linguistically diverse population; good written and oral communication skills; good record-keeping skills; bilingual skills required.
